Safe Motherhood: Our Holistic Approach to Reproductive Health

In the rural landscapes of areas, motherhood is a journey of immense hope, yet it remains shadowed by significant risks. At the Baitambogwe Community Healthcare Initiative (BACHI), we believe that bringing a new life into the world should never cost a mother hers. Since our founding in 2007, we have been dedicated to breaking the cycle of poverty and disease by treating health as a fundamental human right. Our holistic approach to reproductive health is designed to ensure that every woman in the East Central and Greater Masaka regions has access to safe, dignified, and high-quality medical care.

Safe motherhood is more than just a medical goal, it is a cornerstone of socio-economic prosperity. When a mother survives and thrives, her children are more likely to stay in school, her household remains financially stable, and the community grows more resilient. By integrating preventive health services with emergency obstetric care and socio-economic support, BACHI is building a future where Safe Motherhood is a reality for every family.

The Reality of Maternal Health in Uganda

Uganda has made commendable strides in reducing maternal mortality over the last decade. Recent data from the National Population and Housing Census (2024) indicates that the maternal mortality ratio has declined to approximately 207 deaths per 100,000 live births. While this reflects significant progress from the 336 deaths recorded in 2016, it remains far above the global Sustainable Development Goal (SDG) target of less than 70 deaths per 100,000 live births by 2030.

Most maternal deaths in rural areas are preventable. They often stem from the three delays, the delay in seeking care, the delay in reaching a health facility, and the delay in receiving quality care upon arrival. BACHI’s interventions are specifically designed to eliminate these bottlenecks by bringing services closer to the people and strengthening the capacity of local health systems.

A Life-Course Approach to Reproductive Health

Our holistic model recognizes that reproductive health does not begin at the moment of conception. It is a continuum that starts with adolescent health and continues through the post-natal period. By addressing the needs of women at every stage of their life, we create a stronger safety net for both mothers and their children.

Adolescent Health and Family Planning

Uganda has a predominantly young population, with approximately 24% of the population aged 10-19. Protecting the health of young women is critical for the nation’s future. Our programs focus on ending teenage pregnancy and child marriage, which are major drivers of maternal morbidity. We provide family planning services and reproductive health education to empower young women to make informed choices about their bodies and their futures.

Antenatal Care (ANC) and Prevention

Early and regular antenatal care is vital for identifying potential complications before they become life-threatening. According to the Uganda Ministry of Health RMNCAH Sharpened Plan II, initiating ANC in the first trimester is one of the most effective ways to ensure a healthy pregnancy. BACHI works through Village Health Teams (VHTs) to identify pregnant women early and guide them to our health centers for screenings, nutritional support, and HIV testing.

Integrating HIV Prevention and PMTCT

In regions where HIV prevalence remains a concern, safe motherhood must include the Prevention of Mother-to-Child Transmission (PMTCT). Our integrated approach ensures that every pregnant woman is tested for HIV and, if positive, immediately enrolled in life-saving Antiretroviral Therapy (ART).

The UNICEF Uganda 2024 Annual Report emphasizes that integrating HIV services into primary healthcare demonstrated significant progress in 2024, achieving safe delivery rates as high as 90% in supported districts. At BACHI, we strive for a zero-dose generation, where children are born HIV-free and receive all their essential vaccines. This integration reduces the stigma associated with HIV care and makes it a routine part of maternal health.

Empowering Mothers Through Sustainable Livelihoods

We recognize that health does not exist in a vacuum. A mother’s ability to follow medical advice often depends on her financial stability. If a woman cannot afford transportation to a health facility or nutritious food for her family, her health will suffer.

Our Sustainable Livelihoods intervention provides mothers and caregivers with the skills and resources needed to maintain their wellbeing. By supporting income-generating activities and village savings and loan associations (VSLAs), we give women the financial independence to prioritize their reproductive health. This economic empowerment is a critical component of our mission to deliver holistic wellbeing.

Advocacy for Respectful Maternity Care

Every woman deserves to be treated with dignity and respect during childbirth. BACHI advocates for Respectful Maternity Care, ensuring that our medical services are not only clinically sound but also ethically managed. We work closely with district governments and local leaders to hold health providers accountable and ensure that facilities are welcoming, safe, and culturally sensitive.

Our core values of Transparency and Accountability drive us to maintain a gold standard in medical care. We believe that by building trust between the community and the health system, more women will choose to deliver their babies in the presence of skilled health personnel, a key factor in reducing maternal and neonatal mortality.

BACHI serves as a vital bridge between international resources and local transformation. Our work is made possible through strategic partnerships with esteemed organizations such as GAVI, the Global Fund, and USAID/PMI-MRA. By aligning our programs with the WHO recommendations on maternal health, we ensure that the care we provide is based on the latest global evidence and best practices.

These partnerships allow us to innovate and scale our services, reaching the most marginalized women in our districts.
